How Do I Get Started
When your doctor gives you a referral for outpatient physical therapy you have the option to have therapy in the outpatient clinic or in the comfort of your home.
Get an order for physical therapy from your doctor or nurse practitioner and have them fax it to (601) 653-9261 or have them give it to you and your therapist will pick it up when they come out to perform your initial assessment.
Louisiana residents: We can evaluate you and get treatment started without a physician’s prescription. We will send a plan of care to the doctor along with regular updates for their certification.
If you do not have a doctor and feel like you need physical therapy, we can recommend one for you. Simply print a copy of our physician's referral form and take it to the doctor and ask for therapy with Visions At Home Physical Therapy. Have the doctor to sign and fax it to us at (601) 653-9261.
