We are not a Home Health Agency. Home Health Services are covered by Medicare Part A. We are an Outpatient Clinic. Our services are covered by Medicare Part B. Medicare will not pay for Part A and Part B Services at the same time.
If you currently have a Home Health Nurse or Aide coming to your home, we will not be able to provide services for you until your Home Health Services have ended.
You do not have to be Homebound to receive outpatient physical therapy services in the privacy of your home. The homebound requirement only applies to Home Health Agencies.
Please let us know if you are currently receiving or have recently received Home Health Services, so we can verify that they have discharged you from their services.

No! We will bring professional and compassionate care to the convenience of your home for the same cost as going to the outpatient clinic. Medicare and other insurances have always allowed for outpatient physical therapy to be provided in the home. You do not have to be homebound to receive outpatient physical therapy in your home which is a requirement for Home Health Agencies.
Bottom Line: We provide the best physical therapy model to help seniors attain their maximum potential and remain active and safe in their homes.

No, you do not have to use the physical therapy clinic your doctor recommends. While doctors often suggest a clinic that they are familiar with or have had positive experiences with, you have the freedom to choose a clinic that best meets your needs.
Patients have THE RIGHT TO CHOOSE the physical therapy provider they prefer. The patient simply needs to tell the physician or hospital staff that he/she has a physical therapy provider preference and the selection must be honored. The prescription you received is valid at any PT clinic, no matter what PT clinic's name is on the prescription paper.

We can assist with verifying your insurance as coverage may vary!
As a courtesy to our patients, we submit insurance claims electronically on your behalf for quicker reimbursement.

individuals that fall under
one of the following conditions:
You do not have health insurance coverage.
Your health insurance does not cover outpatient physical therapy services.
Your annual limit for outpatient physical therapy services covered under your health plan has been reached.

If you are a Louisiana Resident you have direct access to physical therapy which means that you do not need a doctor’s referral to begin physical therapy treatment. However, some private insurances will not pay for therapy services without a doctor’s referral.
If your insurance provider will not pay for physical therapy services without a doctor’s referral then you will fall under our self-pay program. We would be happy to check with your insurance provider to verify whether or not they pay for outpatient physical therapy services without a doctor’s referral.

Louisiana Residents that do not have health insurance coverage or have exhausted their physical therapy benefits for the year, qualify to receive physical therapy at home without a doctor’s referral. They can simply use our self-pay program to pay for services.

When your doctor gives you a referral for outpatient physical therapy you have the option to have therapy in the outpatient clinic or in the comfort of your home.
Get an order for physical therapy from your doctor or nurse practitioner and have them fax it to (601) 653-9261 or have them give it to you and your therapist will pick it up when they come out to perform your initial assessment.
Louisiana residents: We can evaluate you and get treatment started without a physician’s prescription. We will send a plan of care to the doctor along with regular updates for their certification.
If you have not seen a doctor and feel like you need physical therapy, you can print a copy of our physician's referral form and take it to your doctor and ask for therapy with Visions At Home Physical Therapy. 1. A physical therapist will call you
promptly to arrange a time for
your in-home evaluation visit.
2. Please notify the therapist of your
insurance provider so we can
contact your insurance provider
to verify your benefits.
3. The therapist arrives at the
scheduled time (The therapist
may have portable exercise or
pain equipment to assist with
the therapy session).
4. The therapist will discuss the
following:
Your medical history
Your current problems/complaints
Pain intensity, what aggravates and eases the problem.
How is this impacting your daily activities or your functional limitations?

Medications, tests, and procedures related to your health.
5. The therapist will then perform
the objective evaluation which
may include some of the
following:
Palpation
Range of Motion (ROM)
Muscle Testing
Neurological Screening
Special Test
Posture Assessment
6. Wear loose, comfortable clothing
that allows the therapist to
access the involved area of the
body.
7. The therapist will then formulate
a list of problems you are having,
and how to treat those problems.
8. A plan is subsequently
developed with the patient’s
input. This includes how many
times you should see the
therapist per week, how many
weeks you will need therapy,
home programs, patient
education, short term/long term
goals, and what is expected after
discharge from therapy. This plan
is created with input from you,
your therapist, and your doctor.
9. At the end of your visit,
you can schedule upcoming
appointments for the week
or month with your physical
therapist who will strive to
accommodate your schedule.
10. Therapy sessions vary depending
on your patient needs and
diagnosis, but typically last
40-60 minutes.

We have appointments as early as 8:00 AM for your convenience.
We try to schedule appointments based on your preference, but because we offer an in-home service, we need to factor in travel time and location.
As our caseload varies and location of patients change, so do available appointment times.
Our goal is to be consistent with days and times. We will try our best to accommodate you, but certain days and times cannot be guaranteed for the duration of your treatment program.
By scheduling all your visits in advance, you will be more likely to receive your preferred times.

At Visions At Home Physical Therapy, we understand that life can be unpredictable and sometimes adjustments need to be made to your schedule. To ensure optimal care and make the most of our resources, we kindly ask that you provide at least 24 hours' notice if you need to cancel or reschedule your appointment.
Cancellations or changes made within less than 24 hours may incur a cancellation fee. This policy helps us accommodate other patients who may need urgent care and keeps our operations running smoothly. Thank you for your understanding and cooperation.
